Dr. Bakaly is a Anthem Blue Cross and Aetna Provider and may be listed on other insurance panels, as well. Please let me know at the first session if you plan to use insurance. As a Preferred Provider for Aetna and Anthem Blue Cross of California, I have agreed to work at a contracted rate with this company. If a client chooses to have an insurance company pay for charges for psychotherapy, there are significant confidentiality concerns that the client must understand. Please refer to the insurance section of the Psychologist-Client Services Agreement Form in order to understand these limits and the process of using insurance.
Please do not hesitate to ask Dr. Bakaly in person if you have any concerns about confidentiality when you meet with him. It is the client's responsibility to ensure coverage with Blue Cross and other insurance companies and to provide Dr. Bakaly with the appropriate information required to make the claim. With the client's written consent, Dr. Bakaly will submit regular claims for payment to the insurance company. Co-payment is due at the time of the session.
Good Faith Estimate
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
•You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
•Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
•If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
•Make sure to save a copy or picture of your Good Faith Estimate.
